Enrolling DPT Student Letter of Intent - Class of 2028

This letter of intent formally confirms your intention to enroll in the Northwestern Doctor of Physical Therapy Program starting in the fall of 2025. By submitting this letter, you acknowledge your acceptance of the offered seat and express your commitment to joining the upcoming 100th graduating class of 2028.

By accepting a position in the Northwestern University Doctor of Physical Therapy degree program, Class of 2028 and submitting the $1,000 tuition deposit, I understand and agree to the following:
It is my intent to enroll in the Doctor of Physical Therapy (DPT), Class of 2028 at the Department of Physical Therapy and Human Movement Sciences (PTHMS) of Northwestern University, Feinberg School of Medicine, starting September 2025. I have withdrawn myself from consideration at other institutions and will notify Northwestern immediately if my enrollment plans change. In the event I choose to withdraw prior to enrollment, the tuition deposit will not be refunded. I have read the DPT program’s Technical Standards and am able to meet all of the standards listed. Clinical Education experiences available to Northwestern University physical therapy students are located throughout the United States. I am required to travel a minimum of 50 miles outside of Chicago for at least one of my full-time clinical experiences. Enrollment is contingent upon successful completion of all admission requirements. I must confirm my successful completion of all prerequisite coursework and bachelor’s degree by September 1, 2025. I will do this by either verifying my coursework and/or degree via PTCAS or by submitting official transcript(s) to PTHMS. I will comply with the health requirements requested by Northwestern University Health Service and PTHMS by submitting the necessary forms, immunizations, and lab reports. It is my responsibility to maintain current and acceptable CPR certification for the entire period of enrollment in the DPT program. I will be required to enroll in the NU Student Health Insurance Plan or, if I choose to waive the NU plan, will maintain my own insurance coverage in compliance with University standards. I will be required to demonstrate competency in Medical Terminology by passing an online exam before the start of the Fall 2025 term. I will be required to complete a criminal background check at the start of the Fall 2025 term.
