Class of 2025 DPT Student Scholarship Application

PTHMS is pleased to award several scholarships to incoming and current DPT students. Award descriptions and guidelines can be found here: http://www.feinberg.northwestern.edu/sites/pthms/admissions/dpt/scholarships/index.html

The following awards are available to 1st Year DPT students in the Class of 2025, enrolling Fall 2022.

Please review the instructions below and submit this form by December 3rd, 2021 to be considered for several of the 2022-2023 academic year awards.

1st Year Student Scholarships
(apply by December 3rd, 2021 using this form):
• Early Decision including the Mark Kaufman Scholar (3 $10,000 awards)
• Merit (10 $5,000 awards)
• Sally C. Edelsberg (1 $4,000 award)
• Janet L. Hemzacek Memorial (1 $4,000 award)
• Edna Foster Wright (1 $4,000 award)
• Robert Babbs, Jr. PT Diversity Scholarship (1 $2,500 award)

(application process will occur after DPT enrollment):

EARLY DECISION SCHOLARSHIP

MERIT SCHOLARSHIP Eligibility: My cumulative GPA is at least a 3.6 on a 4.0 scale.

SALLY C. EDELSBERG SCHOLARSHIP

In what specific area(s) do you plan to become a leader in physical therapy/health care? How will you address any barriers/challenges you face? Please describe current or past examples of leadership characteristics or experiences that you have. Please describe what you think a strong leader possesses in terms of characteristics and describe your leadership style.

JANET L. HEMZACEK MEMORIAL SCHOLARSHIP

Please list community service activities in which you participated during college and subsequent years. (Do not include high school activities or volunteer activities intended to prepare for admission to a physical therapy education program.) Please include: name of agency/organization, location, description of your responsibilities, dates of service and hours spent.

ROBERT BABBS, JR. PT DIVERSITY SCHOLARSHIP

Please describe your qualifications for the Robert Babbs, Jr. PT Diversity Scholarship. A student recipient of this scholarship will demonstrate the following:

• Personal characteristics, traits and experiences that will contribute to diversity and excellence in the field of physical therapy

• History of commitment to diversity and inclusion (e.g in your high school, undergraduate institution, faith institution, community, etc)

• How you plan to commit to elevating the status of underrepresented minority individuals in the field of physical therapy
